It's basically assigning all the DVD menu options an identifier. The letter is which "page" of the menu it's on, and the number is which option on the page it selected. Note that the number displayed might not match the actual order shown on screen.

You can split MKV files on a chapter boundary using MKVtoolnix.

I don't have any other Blu-ray drives on hand, but I seriously considered grabbing one when I was struggling with this. However, on close inspection of the non-working discs, they seem to have circle scratches on them, despite all copies being brand new. I'm guessing there is some issue with the way...
