Well the OP in the second report has just now advised that it was a false alarm. The first report is kind of ambiguous as the disc itself appears dodgy, so I wouldn't get too worried about it. The third report is definitely legit though because I can reproduce it. So something was changed between 1....

Exact same issue here with the Region 1 of The Twilight Zone, Season 1 Disc 2, and rolling back to 1.16.7 solves it. I am able to rip the disc in MakeMKV using older version 1.16.7 (hosted at videohelp.com).
