Normal steps to try are

Reboot the drive
Clean the disc with a damp lint free cloth and dry with another.
Replace the disc
Clean the drive lasers
Replace drive with a pioneer with old firmware.

I keep forgetting to test this I test 12+ drives a day I will try to remember to try it out see if I can reproduce it
