Updating the firmware will do nothing to speed up DVD reading. It uses a different laser than BD or UHD, so it doesn't affect it. I should qualify that - if you're seeing riplock with the original firmware (rare!), the new firmware could disable it. But riplock would lock your speed to 2x, not allow...

What year? What does MakeMKV say about it, especially about its UHD capabilities?
The BW-16D1H-U has changed drives over the years. Early ones aren't UHD. Later ones need to have new firmware.

Are you trying to use the key that is "built in" to the file you downloaded, or the one posted in the forum each time it changes? It DOES change every 60 days, which is why it's posted.

Clean the disks. They may look clean, but they need cleaned. Your drive is having trouble reading them.

What it is reading isn't different, so firmware change doesn't help anything. What is possible is that the lens is getting dirty, or the mechanism is getting inaccurate, which firmware won't help. Cleaning the lens assembly is probably the biggest issue, but that's mostly when it stops reading ANY d...

The files are used for MakeMKV to track information it needs for processing the disk. They aren't likely to "be" anything else.
They're created for the Windows version as well. Probably the Mac version too.

No, MakeMKV will always try to give a complete copy of the files. If there are errors, the copy will be rejected.
If you are tolerant of errors, then cleaning the disk to remove the errors is probably your best bet.

MakeMKV isn't intended for working on "downloaded files". It's intended for working with DISKS.
