If you're ripping directly from the disk and MakeMKV cannot generate the proper key, please follow the steps in the FAQ link in my signature so that Mike can look at it.
It is rare that MakeMKV needs to look up a volume key, EXCEPT when dealing with an ISO instead of the actual disk.

But it IS a separate track - it is not needed if you are viewing the movie as 2D. It's just as "connected" as the audio, subtitle, and chapter tracks, in that they work together, but do different things.

That really doesn't narrow it down a lot - I've seen the Heston 'Hur' disk in a couple of different collections.
Disk 2 ripped without error for me back in September without error. It was from a $5 set at Walmart that also had The Ten Commandments (also 2 disks).

As I understand it, the MVC track is the DIFFERENCE between the left/right eyes, and is useless without the "base".

You should always set your MakeMKV data directory (Preferences->General) to something OTHER THAN the program directory. Actually, it says AACS v62, and it saved a copy of the AACS information in the file /Users/MrRobPlus/Library/MakeMKV/MKB_v62_GAMEOFTHRONES_S6_DISC1.tgz . And v62 has been supported since 1.10.2, so it should not be an issue. It may be an issue with permission to talk to the drive directly, but these...

How long is the movie? You've told MakeMKV to not look at any title shorter than 92:45 seconds. If no title is at least that long, you'll get the "failed to open disk".
