Depending upon the nature of the errors, you probably have a dirty or damaged disk, but may have an issue with over-speed on the drive. But, without seeing the debug log, those are just guesses. If the disk isn't being detected in the drive, that is usually caused by the operating system "forgetting" how to talk to the drive. Since this is a USB drive, you can reset the operating system's parameters by unplugging the drive, then plugging it back in after the system notices that it...

MakeMKV copies the MVC track straight through - so if the right/left order is wrong, it was that way on the disk.
It is possible that there is something in the disk menu system that sets options in the player when you select 3D.

MakeMKV doesn't care about regions. And region coding works differently on Bluray than on DVD, in any case. Continuously "loading" can be a sign that the disk is unreadable at the root directory area. I would try cleaning it with a damp soft cloth (cotton or microfiber), and try loading it...

It would appear that the operating system is interfering with access. Have you changed anything at the OS level? Are you running any programs that access the disk, in addition to MakeMKV?
