I've found that when I appended files, chapters with the same name in the second file are discarded, so you have to rename (renumber) the chapters in the second MKV file prior to appending.
Then, in my experience, it's just worked fine straight out of the box.

Enable advanced mode in preferences and you'll get a dropdown alongside the title name which allows you to switch to choosing the filename instead.

Use a different player. Interlaced VC-1 is a problem for a number of players. The MKV file itself is fine.

MakeMKV doesn't set the forced subtitle flag; you have to do it yourself. A lot of players don't handle forced subs very well (some will switch on all forced subs or the highest numbered stream when there is more than one track flagged as forced, for instance) so it extracts them but doesn't flag th...

eac3to (free) can pull out the video track and remove the pulldown flags at the same time. Then MKVtoolnix (also free) can merge the video stream back into the MKV in place of the original stream. Since I've done this to all my files to get true 24p video you might find that the process of demuxing ...

I've ripped 20 or so for playback on the Popcorn Hour and had no real problems. To get 24p video you need to demux, remove the pulldown flags and remux, but that wouldn't stop it playing. And subs cause an issue but they can easily be converted to PGS. One thought though: are you using an AV decoder...

MakeMKV doesn't alter the video or audio streams at all (hence its processing speed) so the quality will be identical to the original source disc.

I think you'll find the majority of people that use MakeMKV want the 30GB files for ultimate quality, but don't want the Blu-ray container or all the extras and other fripperies that purely waste disk space.
It's the same reason why I'd never consider ripping my CDs to MP3.

Is there a checkbox for the MVC extension by the video selection, or something along those lines? The MVC extension is what turns the 2D video into a 3D one. Disable that and you'll be left with a 2D file.
I don't have any 3D Blu-rays either so I haven't seen the option myself.

MKVs don't have region codes, so your player and/or TV won't have an issue with the MKV file unless it can't actually play the particular format of the video (24p, 50i, 60i, 50p or 60p) that's in the MKV file.
