Sure that you are using MakeMKV and not an other tool? MakeMKV does not creatd an compressed MP4 file. This is maybe done by Handbrake or whatever.

I assume that the Offset (Location) is in bytes counting from the start of the rip and not based on the M2TS (Stream Segment) file.

To see more, you must pass the Debug Log which may contain more information about the problem.

No idea, i don't have this Blu-ray.

Check the shown Segment maps for the Playlist (MPLS) in the right MakeMKV window. If they are identical, the Video content is the same. The difference can be within the Audio and/or Subtitle tracks.

I think the lines/dots are the timecode (changes very fast, right?) from the old analog recording area. Old analog TVs did not show it, because the overscan (some lines are shown outside of the screen) removes it. This is a mastering error, the timecode should not be part of the picture on DVD or wh...
