Optical drive "DVD+R-DL HL-DT-ST DVD-RW GH41N PQ06d" opened in OS access mode.
Optical drive "BD-ROM Optiarc BD ROM BC-5500S4 1.Vad" opened in OS access mode.

Then it sounds like you have a disc that is somehow different than the BD+ copy protection that was resolved. I'd suggest you send an email file to the
support@makemkv.com address requesting assistance. I can't imagine another version of the disc with an entirely new disc protection scheme.
