No Errors, but consistent bad rips. Advice?

Ladies and Gents, I've been ripping all of my old and new DVD's and Blu-Rays. Some of them are brand new, some of them are years old. I would say 60-70% of them rip beautifully. But 30-40% of them have chatter, noise, skipping, jumps, etc. I've had to re-rip some disks 2-3 times to get a better rip. Is this due to maybe an older Blu-Ray drive, or something else? It's annoying. I have to watch each MKV for about 5 minutes after ripping just to see if I have to rip it again. I'm using a PC with 8 Core CPU and 32 GB RAM and an SSD, so I don't think it's the PC.
